Immunizations for Students Living or Traveling Internationally
For students entering the United States or who have lived/traveled internationally, below is an outline of immunization and screening requirements required by California State University, Fresno.* If you have questions regarding the form or the university health immunization policy, please contact the Student Health and Counseling Center at 559.278.2734 or visit them during normal business hours.
*IMPORTANT NOTE: Your immunization record must be in English or your immunization record with an English translation will be accepted.

Required Immunization and Screenings | Required Dosage and Screening Information |

Measles, Mumps and Rubella (MMR) |
Two (2) doses with first dose on or after 1st birthday; OR positive titer (laboratory evidence of immunity to disease). |
Varicella (Chickenpox) |
Two (2) doses with first dose on or after 1st birthday; OR positive titer. Note: History of contracting the disease does not meet compliance. |
Tetanus, Diphtheria and Pertussis (Tdap) | One (1) dose on or after age 7. Must have been received within the past ten years. |
Meningococcal conjugate (Serogroups A, C, Y, & W-135) | One (1) dose on or after age 16 for all students age 21 or younger. |
Hepatitis B (Hep B) | Students age 18 and younger (CA Health & Safety Code, Sec. 120390.5) |
Screening/Risk Assessment: Tuberculosis (TB) |
All incoming students must complete the TB Screening Questionnaire in MyBulldogHealth. Incoming Students who are at higher risk* for TB infection, as indicated by answering “YES” to any of the screening questions, should undergo further TB testing. *Higher risk includes travel of living in South and Central America, Africa, Asia, Eastern Europe, and the Middle East; prior positive TB test; or exposure to someone with active TB disease.
