Application to the SSSV Program is now open!

STEP 1: Make sure you meet all eligibility criteria

STEP 2: Submit an online application: APPLICATION FORM

• Within the application, prepare to submit the following:

  • Most recent or 2022 Income Taxes OR an Income Self-Reporting Form (if you don't have your Income Taxes)

  • DD-214 form 'Member 4 copy'. For National Guard: NGB Form 22 

  • VA Benefits Summary Letter (If applicable) 

STEP 3: Successful applicants will be notified via email for an Intake appointment.
